1. Program Name & Service Description
KLEO Notifications is the official administrative messaging program for KLEO Community Family Life Center. KLEO is a clinical mental health practice providing therapy, counseling, and care coordination services through in-person and HIPAA-compliant virtual platforms.

2. Emergency & Crisis Disclaimer
KLEO DOES NOT PROVIDE 24/7 CRISIS SERVICES.
SMS, email, and online communication tools are for administrative coordination only and are not monitored in real time. If you are experiencing a life-threatening emergency, call 911 or the 988 Suicide & Crisis Lifeline immediately. Do not use SMS or email for urgent or emergency clinical needs.

3. SMS & Electronic Communications
By providing your mobile phone number and opting in, you provide prior express consent to receive SMS, MMS, and voice communications from KLEO through authorized service providers.

Coverage information is not a guarantee of payment. Clients are responsible for non-covered services.

Authorization Requirements
Some services require prior authorization. Services may be paused if not approved.

Eligibility Responsibility
You are responsible for maintaining active insurance coverage.

Financial Responsibility
You are responsible for copays, deductibles, and non-covered services.

Cancellations
Late cancellations or no-shows may incur fees and are not billable to insurance.

Billing Corrections
KLEO may correct billing errors and resubmit claims when necessary.

Care Coordination
We may coordinate care with other providers when appropriate and authorized.

6. Telehealth Services
Telehealth services are provided via secure platforms. By participating, you acknowledge:
- Technical disruptions may occur
- You must be located in a state where your provider is licensed

7. Limitation of Liability
KLEO is not liable for indirect or consequential damages arising from use of communication services or the website. Any legal claim must be brought within one (1) year.

8. Privacy & Data Protection
KLEO does not sell personal information for marketing purposes.
Third-party providers may be used for operations and are required to safeguard your data. Text messaging opt-in data will not be shared outside of messaging providers.

9. Governing Law
These Terms are governed by the laws of the State of Illinois.

10. Changes to Terms
KLEO may update these Terms at any time. Updates will be posted with a revised effective date.
